The LAFD Membership Application may be printed out and mailed or faxed. Please note, however, that your membership cannot be processed until the appropriate fees have been received and processed and your resale certificate verified with the California Board of Equalization.

Two types of printable forms are provided:

• An HTML form, a text form which can easily be printed from any computer
